- Shannon and Lee for obtaining OASIS certification
- Charlie Moore, (PT) who received a letter from patient saying “he’s like the brother I never had”
- All nurses — great team, always help out, dedicated to their jobs
- Brian Leutner — thanks for finding us space at Dorchester for staff to work from
- Reimbursement Team — they do a great job of collecting the money

- Bridgette Wilson, Margaret Wood and Ashley Deiter – all three create a good rapport w/patients
UM Chester River Home Care:
- “All staff, we work well as a team”
- The camaraderie was evident in the following: Monthly team breakfast for staff meetings where each team member brings in his or her specialty; fundraising events to support patients in need of self-pay items; office is clean, tidy; team members take pride

Shore Home Care
- Return calls promptly
- Develop rapport with patients from the very beginning
- Provide contact information for patients after discharge so they don’t feel deserted
- Make patients feel like they are the only patient
- Coordinate care among disciplines
- Provide same day services (LifeLine)
Requard Center for Acute Rehabilitation:
- Educate patients and families on what to expect prior to or at admission to the unit
- All disciplines work well as a team
