Everything You Need to Know About Flu Shots: October 2-November 2, 2017

 
Annual Flu Shots are a mandatory condition of employment for all staff (unless there is a medical contraindication).  In that case you will need to sign a declination form and provide a note from your medical doctor (if a current one is not on file).  If you are pregnant you will need a note from your OB provider before receiving the vaccination.  Declination forms may be obtained from your manager, on the Intranet or directly from Employee Health.  (Note: Compliance date is November 3, 2017)
You may attend any one of clinics below – please note there will be no flu vaccinations given at the Employee Health offices. EHS staff request encourage all staff to get their shot EARLY — don’t wait until the last minute!

Other Reminders:

  • The flu vaccination is a — Regular dose vaccine
  • Bring your Employee Badge
  • Wear short sleeves or loose fit shirt, if possible.
  • Egg free vaccine will be available upon request.
  • If you have a valid Declination, you will need to wear a mask from October 1 until April 1, 2018 while on duty, per UM SRH policy.
  • If you received the vaccination elsewhere please send proof of such to Employee Health as soon as possible.
