As the survey was released, some team members responded that their psychological safety could be enhanced by making the question related to identifying your direct supervisor as optional. In light of this feedback, if you do not feel comfortable giving the name of your direct supervisor please enter “n/a” for question #1 or consider entering one of the more broad categories that reflect where you work, for example: “patient care services” “operations” “support services” “medical staff” “quality/safety/experience” “administrative support” “clinical and ancillary services” or “outpatient practice” instead.
