Influenza
The SRH flu vaccination campaign kicked off October 2. The deadline for compliance with the policy is November 2.
SRH’s flu policy HR-70 requires that all individuals providing care, treatment or service to the patients at Shore Regional Health (SRH) are required to be vaccinated annually against influenza unless there is a medical contraindication or a religious reason for declining vaccination. Clinic locations and hours are as follows:

Reminders:
- The flu vaccination is a — Regular dose vaccine
- Bring your Employee Badge
- Wear short sleeves or loose fit shirt, if possible.
- Egg free vaccine will be available upon request.
- If you have a valid Declination, you will need to wear a mask from October 1 until April 1, 2018 while on duty, per UM SRH policy.
- If you received the vaccination elsewhere please send proof of such to Employee Health as soon as possible.
Vaccine Declination
Under the policy, all employees must be vaccinated; declining is allowed for medical or religious reasons or if you received the vaccine from a source other than SRH Employee Health.
Anybody who declines vaccination, or who gets the vaccine elsewhere, must complete a Declination Form and submit it, along with the required documentation. The deadline for submitting the required documentation is November 9th, 2017.
