HR News You Can Use: Leaves of Absence


Liberty Mutual is our partner and administrator for all leaves of absence. If you anticipate a leave of absence — and even if you expect to be out for less than two weeks — you must contact Liberty Mutual to begin a claim. This is the case if you …

  •  have a chronic medical condition that causes you to miss time from work
  • are scheduled for a surgery;
  • have a serious injury or illness that causes you to miss work

Initiating a claim through HR Connections:

  • Log into HR Connections using your ID and password.
  • Once in the Knowledge Center, click on the quicklink for “Disability, FMLA and other Leaves”  — that will take you directly to mylibertyconnection.com.  This site provides an easy and secure way to report your disability claim.
  • Please select the “Report a New Claim” button on the home page of this site. Since your absence may also qualify for job protection under the Family and Medical Leave Act (FMLA), the information you provide to process your disability claim will also be used to initiate a Leave of Absence request, if applicable.

Initiating a claim by phone: You may also call Liberty to start your claim at 844-208-1665.  It is extremely important that you follow all requirements that the Liberty representative explains during the call.  You also must stay within the time frame allotted for documentation to be completed and returned to them for your claim to be considered and processed.
If you are approved for intermittent leave (leave that is not consecutive days but sporadic): You will be required to report all leave-related absences to Liberty Mutual as well as to your manager. A few things to note:

  • You will be asked the dates and hours missed from your scheduled work.
  • There is a time limitation on the reporting, so absences need to be reported as soon as possible after they occur.
  • You may enter the absences on the website using the same procedure as above or you may report the time missed by phone.

If you need a leave of absence that is not for medical reasons: You must also contact Liberty Mutual to initiate.  Please discuss this need with your manager first, as he or she must approve any request that is outside of the conditions of approval for medical leaves.
