Name Change due to Marriage or Divorce? You must complete a “Personal Data Change Form” found on HR Connections and submit in a case. The only acceptable document as proof of the name change is an updated Social Security Card. This should be attached to your submitted case, along with your marriage certificate or divorce decree.

Need to update your taxes or Direct Deposit Information? We are fully automated! All forms must go through HR Connections Employee Self-Service. This will allow you to update your information quickly and confirm you changes via email. No paper forms will be accepted by Payroll or Human Resources, effective March 1, 2018.
